These Terms & Conditions govern your access to the Golden Assistance Clinic website and any telehealth services we provide. By using the site or engaging the Clinic as a patient, you agree to these Terms. If you do not agree, please discontinue use of the site and do not establish care with us.
Who We Are
Golden Assistance Clinic is a telehealth primary-care practice licensed to provide care to patients physically located in Florida and New York. Clinical care is delivered by an Advanced Nurse Practitioner credentialed in both states.

Telehealth Services and Eligibility
Telehealth services are available only to patients who are:

- Physically located in Florida or New York at the time of the visit.
- Able to complete identity verification and an intake form.
- Engaging the Clinic for a condition appropriate for virtual evaluation, in the clinician's professional judgment.
Establishing care does not guarantee any particular treatment, prescription, or outcome. Your clinician will exercise independent clinical judgment in every visit and may recommend in-person evaluation when appropriate.
Patient Responsibilities
You agree to:
- Provide accurate, complete, and current information about your health.
- Verify your identity and physical location when requested.
- Use the telehealth platform only for your own care.
- Follow the care plan agreed with your clinician, including taking medications as prescribed and completing recommended labs.
- Notify the Clinic promptly of any changes in your condition, medications, or contact information.
Appointments, Cancellations, and Fees
Appointment scheduling, fees, accepted insurance, cancellation windows, and no-show policies are communicated at booking and may be updated from time to time. Please contact our office before your appointment to verify coverage and understand any patient responsibility.
Prescriptions
Prescriptions are issued at the clinician's discretion based on a documented clinical evaluation. We do not prescribe controlled substances through telehealth except where state and federal law allows and an in-person standard of care can be maintained. Refills are subject to clinical review and routine follow-up.
Use of the Website
You agree to use the website lawfully and responsibly. You may not:
- Misuse, interfere with, or attempt to gain unauthorised access to the site or its systems.
- Submit false, misleading, or harmful content through any form.
